Before-and-after photos on Instagram are curated to sell products. Clinical trial data tells a different — and more useful — story. Here's what the research shows you can realistically expect at each milestone, organized by condition.
Skin Rejuvenation: Before & After Timeline
Week 2
- What you'll notice: Improved "glow," slightly better skin tone, temporary plumping effect
- What's happening: Increased blood flow and cellular metabolism. No structural changes yet.
- Clinical data: Most studies report subjective "skin feels better" at this stage. No measurable collagen changes.
Week 4-6
- What you'll notice: Smoother texture, reduced redness (if rosacea or inflammation), subtle improvement in fine lines
- What's happening: Early collagen synthesis beginning. Fibroblast activity increasing.
- Clinical data: Wunsch & Matuschka (2014) began seeing statistically significant texture improvements at 6 weeks with 633 nm, 3x/week.
Week 8-12
- What you'll notice: Visible reduction in fine wrinkles, improved firmness, more even skin tone
- What's happening: Measurable increase in collagen density. Extracellular matrix remodeling.
- Clinical data: Wunsch & Matuschka measured 26% increase in collagen density by profilometry at 12 weeks. Ablon (2018) reported 36% improvement in wrinkle scores at 12 weeks.
Week 16+
- What you'll notice: Continued improvement, though rate slows. Maintained results with 2-3x/week maintenance.
- What's happening: Collagen maturation and cross-linking. Long-term structural improvement.
Hair Growth: Before & After Timeline
Week 4-8
- What you'll notice: Little visible change. This is the most discouraging phase.
- What's happening: Follicle metabolism increasing, anagen phase extending, but new growth isn't visible yet.
Week 12-16
- What you'll notice: New vellus (fine, baby) hairs appearing. Existing hair may feel thicker.
- Clinical data: Lanzafame et al. (2013) measured 39% increase in hair count at 16 weeks. Jimenez et al. (2014) saw 20+ additional hairs/cm² at 16 weeks.
Week 20-24
- What you'll notice: Vellus hairs converting to terminal (thicker, pigmented) hairs. Visible improvement in density.
- Clinical data: Most studies show continued improvement through 24 weeks, with some reporting peak results at 26 weeks.
Week 24+
- What you'll notice: Stabilization of results. Maintenance protocol needed to sustain gains.
- Clinical data: Discontinuation studies suggest some regression after stopping treatment, but baseline is typically higher than pre-treatment.
Pain & Inflammation: Before & After Timeline
Session 1-3
- What you'll notice: Temporary pain reduction lasting hours to a day. This is real but transient.
- What's happening: Endorphin release, increased blood flow, temporary nerve modulation.
Week 2-4
- What you'll notice: Cumulative pain reduction. Pain-free intervals getting longer.
- Clinical data: Chow et al. (2009, The Lancet) meta-analysis showed significant pain reduction for chronic neck pain by week 2-4 of treatment.
Week 4-8
- What you'll notice: Substantial improvement. Many patients report 50-80% pain reduction.
- What's happening: Tissue repair, reduced inflammatory mediators, improved joint lubrication.
- Clinical data: Knee osteoarthritis studies show peak improvement at 6-8 weeks of 3x/week NIR treatment.
Muscle Recovery: Before & After Timeline
Per Session (Pre-Exercise)
- What you'll notice: Reduced perceived exertion, slight strength improvement
- Clinical data: Ferraresi et al. (2012) showed improved muscular performance when RLT was applied before exercise.
Per Session (Post-Exercise)
- What you'll notice: Reduced DOMS (delayed onset muscle soreness) the next day
- Clinical data: A 2018 meta-analysis showed PBM reduced creatine kinase levels (a marker of muscle damage) when applied post-exercise.
How to Photograph Your Progress
If you want to track your own before-and-after:
- Same lighting, same angle, same time of day — morning light before any products
- Use your phone's timer or a mount — handheld selfies change angle each time
- Photograph every 2 weeks — weekly changes are too subtle to see
- Use the same camera settings — auto-exposure and filters will distort results
- Be patient — real structural changes take 8-12 weeks minimum
