There is no NCD for neurofeedback (EEG biofeedback). Standard biofeedback coverage (NCD 30.1) applies to muscle re-education only, not EEG-based neurofeedback. Coverage is at MAC discretion — most Medicare Administrative Contractors do not cover neurofeedback for ADHD, anxiety, depression, or other conditions. Some LCDs may exist in specific jurisdictions.

Private Insurance Coverage
Not covered by Medicare. Some private insurers cover when billed as psychotherapy or biofeedback by a licensed provider. Prior authorization usually required.
